For elderly people and their families, facing the future today has never been less predictable or more complex.

The simple truth is that we are living longer as a society, which is good. But with longevity – and the enduring comforts of family, friends, loved ones, and the everyday pleasures of being alive – come challenges that unfold on many fronts.

My message to you is a simple one. SeniorBridge exists to help people understand, manage, and master the difficulties of old age. Our work centers on those with complex and chronic medical conditions. And our goal is to keep them safe and cared for in the comfort of their own homes.

SeniorBridge care is based on an integrated practice model. Our company’s unique approach addresses the total well-being of our clients and their families through a comprehensive program that includes assessment, planning, service coordination, advocacy, and direct care by an interdisciplinary team led by a geriatric care manager. Without the type of care offered by SeniorBridge, people with chronic conditions would be placed in nursing homes, dementia units or other type of long-term care institutions.

I believe this is true because our work at every level is based not only on delivering care, but on sharing information.
