The SeniorBridge care team provides a full spectrum of services needed to help elderly or disabled people stay comfortably and safely in their homes. Our inter-disciplinary team of nurses, social workers and in-home caregiver professionals provide high quality care that leads to total patient and family satisfaction and improved outcomes.

Executive Director

The Executive Director is responsible for the growth, development, and management of the business at our branch locations. The Director is a senior leadership position for our branches, ensuring that the SeniorBridge brand remains a trusted, high-quality and valuable part of the community. The Director combines superior operational skills with strong relationships with community and professional organizations to create a consistent network for ongoing client referrals and employee recruitment.

Director of Client Service (RN and MSW)

The Director of Client Service is the team leader for Client Services and responsible for clinical excellence. The Director is responsible for the quality and overall appropriateness of service delivery to all clients, and for maintaining a high level of client satisfaction. This person will be knowledgeable about and compliant with all applicable laws and regulations to which SeniorBridge may be subject. By ensuring high-quality patient care standards with exceptional service, the Director will generate ongoing “word-of-mouth” referrals to both clients and potential employees.

Director of Professional Relations

The Director of Professional Relations is responsible for managing and developing active relationships with leading referral sources and influencers in the community to increase referrals of qualified clients to the Company. The Director works with prospective clients to help transition them into SeniorBridge clients. This person also networks with various medical, legal, financial, and other community resources to develop mutually beneficial relationships. The Director is viewed as a local expert in the field of eldercare and related issues of aging.

Care Manager (RN and MSW)

Our Care Managers provide direct care to clients and their families according to an established plan of care. A Care Manager is the primary contact for our clients and nurtures their long-term relationship with SeniorBridge. Care Managers will develop, implement, and execute a customized optimal care plan for each client. They identify appropriate healthcare resources; coordinate medical treatment; provide supportive counseling to client and family; develop and implement emergency plans; plan long-term training, support, and supervision for our in-home caregivers who provide the direct service, and many other activities; and act and advocate in our clients’ best interests.

Personnel/Scheduling Coordinator

The Personnel/Scheduling Coordinator is responsible for carrying out the day-to-day personnel recruiting, paraprofessional scheduling, and general office functions of the branch. Coordinators are administrative staff who work directly between the professional and paraprofessional staff. Possessing strong communication skills, they spend time interviewing, evaluating, and managing documents to ensure smooth delivery of services.

On-Call Scheduler

This position is similar to that of the Personnel/Scheduling Coordinator, with the difference being that the On-Call Scheduler is expected to work at home when SeniorBridge offices are closed.
