What is a Geriatric Care Manager?


Our Geriatric Care Managers are Social Workers and Nurses who specialize in caring for the elderly. They assist the older individual with disabilities to attain maximal functional potential. A key part of their role is to provide the needed care coordination with physicians, eldercare professionals, and support services to furnish the type of help each person needs. In addition, our care managers also provide specific disease management for the person’s chronic medical condition(s).

What is an In-Home Caregiver?


SeniorBridge provides specially trained home health aides to support continuous in-home care for the elderly person who is unable to perform daily living activities. Our Caregivers are hand-selected and trained to offer the support each person needs, day in and day out. Both personal care and household management are part of the service.
