What is a Geriatric Care Manager?
Our Geriatric Care Managers are highly trained and experienced Social Workers and Nurses who specialize in caring for for older adults and persons with chronic disabilities. They assist the older individual with disabilities to attain maximal functional potential. A role is to provide the needed care coordination with physicians, eldercare professionals, and support services to assure the type of help each person needs. In addition, our care managers provide specific disease management for the person’s chronic medical condition(s). The care manager uses his/her skills to create a trusting relationship with all members of the family and care team in order to create an individualized plan of care specific to the needs and preferences of each situation.
What is an In-Home Caregiver?
SeniorBridge provides specially trained home health aides to support in-home care for the older adult who is unable to perform daily living activities. Our Caregivers are hand-selected and trained to offer the support each person needs, day in and day out. Both personal care and household management are part of the service. Our highly skilled Care Managers oversee all aspects of in-home caregiving on an ongoing basis.
