Our Geriatric Care Management team enables seniors to receive care in the comfort of his or her own home or any setting. Our programs are available 24 hours a day, 7 days a week, 365 days a year.
The SeniorBridge approach to care is based on our own Integrated Practice of licensed nurses, licensed social workers and certified caregivers working together to provide you with comprehensive care. We begin with a thorough assessment of your needs, including medical, functional, psychological, social and financial issues. Based on this assessment, licensed nurse and social worker care managers develop a comprehensive care plan that is personalized to meet the unique needs of each client. Our personalized service program allows families to select just the right care for their specific needs and circumstances. SeniorBridge Care is distinguished by the following services:Dedicated Geriatric Care ManagersClients are assigned a Geriatric Care Manager (a registered nurse and/or masters level social worker) who develops a plan that is customized to their needs and oversees all the care.Tele-health MonitoringSeniorBridge nurse care managers coordinate treatment of our client’s diseases, including using in-home monitoring of a client’s clinical condition.Certified At-Home CaregiversWe employ only those caregivers who are certified and highly experienced. Our rigorous screening process includes credit, drug, and criminal background checks. We hold extensive personal interviews, which include role-playing conducted by our Geriatric Care Managers. We select only individuals who we would be willing to trust in our home.Superior SupervisionEvery at-home caregiver is oriented, supervised and guided continuously by a Geriatric Care Manager. The supervision ensures that our clients are getting exactly the care they need.Comprehensive Services 24x7Because we know that a problem can occur any time of the day or night, we are staffed to give you 24x7 care.Unsurpassed Level of CustomizationEvery client receives their own plan of care, exactly suited to their needs.
