SeniorBridge is a Provider of Comprehensive Care ManagementWe are a highly specialized company with extensive knowledge and experience in managing the care of people who have complex chronic conditions, in their own homes and in other settings. Our program provides quality care for seniors with multiple chronic conditions such as congestive heart failure, chronic obstructive pulmonary diseases, arthritis, diabetes, mental illnesses, Alzheimer's disease, and other dementias. Our model offers an integrated team of nurses, social workers, therapists and physicians, and home aides. Our services address a full range of holistic needs, including a client's medical condition and longer-term issues such as the client's comfort, emotional well-being, functionality, nutritional and psychosocial needs and household management.Our Clinical Model is based on an Integrated Practice Unit (IPU)SeniorBridge provides comprehensive care through an interdisciplinary team led and managed by a geriatric care manager who is a nurse or licensed social worker. This approach allows people to receive ongoing, continuous, integrated care in their homes and coordinates all services and medications from SeniorBridge staff as well as from Medicare providers.
