SeniorBridge Convenes Symposium of Leading Eldercare and Aging Experts to Develop Resource Guide to Provide Support for Physicians in Coordinating Complex Chronic Care Management

(NEW YORK CITY, NY) May 7, 2008

On April 25, SeniorBridge assembled its Professional Advisory Board, comprised of leading experts in the field of aging and chronic healthcare, to develop a set of best practices for improving the care and management in the community of patients with clinically complex, chronic health and psychosocial situations. The group, whose expertise spans geriatric medicine, geriatric care management, social work and elder law, focused on outlining a process for identifying and prioritizing patient needs in outpatient and physician office settings. The result of the Professional Advisory Board’s input will be a resource guide that will help physicians and other healthcare professionals provide and/or coordinate the best possible care for their frail elderly patients. There will also be a version of the guide for consumers and caregivers to help them establish support systems for themselves and those they care for.
