(NEW YORK CITY) June 11, 2008 - SeniorBridge, a specialized organization that serves seniors and their families by managing complex, chronic care in the home and other settings, announced that seven members of its nursing/care management staff have received certification from the National Academy of Certified Care Managers (NACCM). They may now use the designation CMC, Care Manager, Certified.

The certifications complete the first phase of a companywide program to certify all care management staff. The purpose of NACCM is to advance the quality of care management services by assuring individual competence to perform the full range of care management tasks through a validated, standardized examination that tests the skills, knowledge and practice ethics needed to serve consumers.

Care management services link and coordinate assistance from both professional service providers and family and friends to enable consumers with functional limitations to obtain the highest level of independence consistent with their capacity and care preferences. NACCM-certified care management must include face-to-face interviewing, comprehensive assessment, care plan development, implementation and monitoring, problem-solving, reassessment and quality evaluation. It encompasses various settings, target populations, program designs, and levels of duration and intensity of the care management services. SeniorBridge focuses on care management for the frail elderly.

Schmidt RN, CMC, BSN, MPA - Director of Client Service.
