Chronic health issues create a very stressful situation for both the care recipient and the family. Our specially trained Care Managers interact with the care recipient, family, friends, and other members of the community to help mediate conflict, keep family members informed and involved, and encourage social stimulation for the older person. Areas of focus include:
  • Education to help everyone understand what is happening plus how to best meet the needs of all family members
  • Help with problem solving around options for care
  • Conflict mediation to gain consensus for the most appropriate plan of care
  • Discussion support to insure that families are making the best choices they can for their individual circumstances
  • Support for education for managing behavioral problems that arise due to dementia or mental health issues
  • Coordinated care advocacy for treatment of symptoms of mental health problems including abusive behaviors
  • Reduction of social isolation through the introduction of specialized activities companions or the use of community services
  • Accompaniment to appointments and to activites
  • Working closely with clients to overcome resistance to care
