Barbara Hornby, RN, BSN, PHN, CCM
Executive Director

Before joining SeniorBridge, Barbara owned her own Care Management company for 7 years. Prior to that, she was the Director of Resident Services at an assisted living community, Nursing Supervisor at an intermediate and long-term care facility, and a Liaison Nurse for an acute rehabilitation hospital. Barbara started in Psychiatric and Public Health nursing over 30 years ago. She attended USC and graduated magna cum laude from Mount Saint Mary's College in Los Angeles. She moved to Connecticut over 20 years ago, loves New England, and has dedicated herself to helping seniors live the highest quality of life possible. In addition to having a Bachelor's Degreee in Nursing and a Public Health Certification, Barbara is also a Certified Case Manager (CCM).

Kathryn Bartels, RN
Care Manager

Kathryn most recently served as a private-duty home care nurse for seniors and as a nurse for a primary care physician offering "concierge" medical services in Guilford. For 10 years, she was weekend supervisor at Evergreen Woods Health Center, a 50-bed facility in a continuum of care center in North Branford. Previously Kathryn had been a Per-Diem, Per-Visit Nurse for VNA Community Health Care in Guilford, planning and providing continuity of care between primary care physicians and caregivers. She was also Assistant Director of Nursing Services and Charge Nurse of the Sub- Acute/Rehabilitative Unit of The Madison House, a 90-bed skilled nursing facility. Earlier, Kathryn had been Unit Manager/Head Nurse of the Arden House Nursing Home in Hamden. She earned her RN/ASN degree at Quinnipiac College, Hamden, and is licensed as a Registered Nurse in Connecticut.

Maureen MacGregor, BS
Care Manager

Maureen brings 15 years experience in social work to SeniorBridge. In addition to the past 4 years working with Barbara Hornby at Senior Care Consulting, Maureen has worked with the state of Connecticut as a social worker in the Department of Children and Families and the Department of Mental Health and Addiction Services. Maureen has an Associates degree in Human Services from Gateway Community Techinical College, a Bachelors degree in Community and Human Services from SUNY-Empire State College and is currently pursuing a graduate certificate in Geriatric Care Management at University of Florida.

Ruth Farrago, CMC
Vice President of Operations

Ruth Farrago has over 20 years of experience in health care administration. She was the Administrator of an inpatient and outpatient comprehensive Rehabilitation and Orthopedic Division at Downstate Medical Centers in Brooklyn. She has an extensive background in home care marketing and management having worked for St. Vincent’s Catholic Medical Centers as the Director of Intake and was then the Branch Manager/Administrator at New York Health Care. Ruth was also the Executive Director of Sunrise Assisted Living of East Meadow where she was responsible for the daily operations of the facility, client and family satisfaction, management of the sales and marketing team and coordinated community outreach and events. Ruth has a Bachelors of Science from SUNY College of Oneonta where she majored in Business Economics and she is currently completing a Masters Degree at Long Island University specializing in Gerontology. Ruth has received her certification in Geriatric Care Management (CMC). She is a current member of the National Association of Professional Geriatric Care Managers and the Case Management Society of America.
