Sydney K. Lenz, RN, CMC, GCN Executive Director / Director of Client Services

A nurse since 1991, Sydney has specialized in home healthcare for the past 11 years.
Prior to coming to SeniorBridge, she was Regional Director of Texas Home
Health/Guardian Home Care Holdings, with responsibility for programs, operations, and fiscal management in the San Antonio and Kerrville offices. Sydney was also
previously Director of Patient Care at Girling Health Care, Inc, and Executive
Director/Administrator and, earlier, Clinical Manager/Administrator of Baptist Home
Health Services in San Antonio. She received her degree from Baptist Hospital School
of Nursing in San Antonio.
Sydney Lenz has completed the Guided Care Nursing Course developed and offered by the Institute for Johns Hopkins Nursing and has been awarded a Certificate in Guided Care Nursing, endorsed by the National Gerontological Nursing Association.
Isaac A. Kadane Professional Development Specialist
Isaac's entrepreneurial, leadership, and customer service skills should serve
SeniorBridge well. Previously, Isaac was President and CEO of Alamo Infusions, LLC,
in San Antonio, a multimillion-dollar company that provides outpatient infusion
services to chronic and geriatric patients in physicians' offices. Earlier, he was a
Marketing Representative for Federated Mutual Insurance in Fort Worth, where he sold
a full range of financial products and insurance to group health and property and
casualty accounts and won awards for the highest performance standards. Isaac earned
a BA with a major in Corporate Communications and Public Relations with a
concentration in Business Marketing and Management,
from Baylor University in Waco.
Melinda (Mindi) Silver-Weiss, MSW, LCSW Senior Care Manager
For more than 27 years, Mindi has been a social worker involved in varied facets of healthcare. She was
formerly a crime Victim Advocate for the Jewish Family Service in San Antonio and
also Associate Director of the Dorothy & Peter Brown Jewish Community Adult Day Care
Program in West Bloomfield, MI, supervising staff and providing services to
participants with dementia and their families. Previously, Mindi also served as
Supervisor, Senior Services, and Clinical Social Worker at the Jewish Family Service
in Southfield, MI. She has been a Nephrology Social Work Coordinator at the
Southfield Dialysis Facility and Nephrology Social Worker at William Beaumont
Hospital in Royal Oak, MI, and Hutzel Hospital in Detroit. Mindi received a BA in
Sociology/Social Welfare from Wayne State University and her Master of Social Work
degree from the University of Michigan School of Social Work, Ann Arbor. She is a
member of the Academy of Certified Social Workers, and is licensed in Michigan and Texas.
John Anderson, LMSW Care Manager
John has worked for social service organizations for more than 24 years. Since 2003,
he has been a psychotherapist with Senior Connections of Texas. Previously, he was a
hospital-based Adult Protective Services Specialist for the Texas Department of
Protective and Regulatory Services, coordinating APS client services with with the
University and VA hospitals. Earlier, he investigated reports of abuse and neglect of
elderly and disabled clients for the state and consulted on improving quality and
intensity of client services. John has experience as a Social Worker serving
community-dwelling elderly and disabled adults, CPS worker, child care licensing
representative, Medical Specialist and Instructor in Psychiatric Procedures for the
US Army at Brook Army Medical Center at Fort Sam Houston. He completed a Bachelor of
Arts degree in Psychology at the University of Florida, Gainesville, and has been a
Licensed Master Social Worker for over 20 years.
Tami Lykins, RN Care Manager
Ms. Lykins worked at McKenna Memorial Hospital in New Braunfels as a Charge Nurse and Surgical Floor Nurse and is a certified Chemotherapy Provider. She also spent 15 years managing several businesses as well as traveling as a Regional Training Director, recruiting, hiring, and training all levels of employees. She started Customer Service Consultants to help businesses strategize to improve customer relations. Ms. Lykins received her RN from the San Antonio College School of Nursing.
David Mattioli, RN, CMC, BSN Vice President, Regional Operations

David Mattioli brings more than 17 years' experience in a variety of healthcare
settings to SeniorBridge. For 9 years previously, he was the Administrator of Almost
Family in West Palm Beach, responsible for operations and delivery of care for the
in-home program in 4 counties, and for a time served dually in that role at Almost
Family's Medical Adult Day Center in Boynton Beach.
Mr. Mattioli began his nursing career at St. Joseph Mercy Hospital in Ann Arbor,
Michigan, where he developed a passion for caring for the elderly, and became a care
manager before returning to Florida. He is a certified trainer for the Alzheimer
training course for home health agencies in Florida and coauthored the curriculum
used for the training. Mr. Mattioli is a member of the National Association of
Professional Geriatric Care Managers. He received a BS in biology from Palm Beach
Atlantic University and a BSN from Eastern Michigan University, and is a certified
Legal Nurse Consultant. He is a current member of the National Association of Professional Geriatric Care Managers and the Case Management Society of America.
